INSTRUCTION:
After accomplishing this form, submit the same at your preferred Examination Center together with the following: 2pcs (2x2) ID pictures in white background, authenticated copies of one (1) NSO Birth Certificate and one (1) Transcript of records. Original copy will not be accepted (All documents shall become PAFPMC property ). Walk-in applicants may be accommodated provided that they bring with them their 2pcs 2x2 ID pictures in white background , original and authenticated copies of NSO birth certificate and Transcript of Records.

Male or female natural born citizen of the Republic of the Philippines At least 72 College Units Height of at least 152.4 cm (50) for both male and female Not less than 18 y/o and not a day older than 23 y/o upon admission for training Single, never been married and no legal obligation to support a child or children Physically and mentally fit for military training With good moral character
