MCAT or Equivalent Result Card Domicile Certificate Fitness Certificate in original by a Registered Medical Practitioner / Government Medical Officer, with PM&DC Registration number certifying that the candidate have been fully vaccinated against Hepatitis and Tetanus Challan Form/Bank Draft of Rs.1000/(Processing Fee, non-refundable) in favor of Treasurer UOS. IV. For admission to MS/MPhil/PhD Programs (only) GRE (NTS) General GRE Subject (International) MS/MPhil/MD or Equivalent MSc(Hons) MA/MSc PharmD MBBS/BDS BA/BSc (Hons) Certificate of experience from the employer. Printed copy of the Publications/ Research Papers Letter of permission from the employer (If applicable)
