BAR/BAT MITZVAH CONTRACT

I/We request the date of ___________________________________ at 9:00 a.m.

For my/our child(ren), ______________________________________ to be called to the

Torah

as Bar/Bat Mitzvah or B’nai Mitzvah.

I/We understand that the Rabbi must confirm this date, and I/we must meet the following
financial responsibilities:

Full Payment of the Bar/Bat Mitzvah fee of $1,300 is due in full twelve (12) months
prior to the Bar/Bat Mitzvah date. Payment of $750.00 for the Sponsorship of the
Saturday Morning Basic Kiddush is due and payable 2 months prior to the Bar/Bat
Mitzvah date.

1. A non-refundable deposit of $300.00 must accompany this form to

hold the above date. Neither this deposit nor any other payment of
the Bar/Bat Mitzvah fee is applicable towards other Temple Beth
Sholom payments.

2. $1000 is due twelve (12) months prior to the Bar/Bat Mitzvah date.

3. The $750.00 Sponsorship fee of the Shabbat Morning Basic Kiddush

is due and payable two (2) months prior to the Bar/Bat Mitzvah date.
Please see “catering” section for “Basic Kiddush” specifics.

4. All Synagogue dues and tuitions must be current and remain current
through the Bar/Bat Mitzvah date.

The $1,300.00 Bar/Bat Mitzvah fee includes:

Bar/Bat Mitzvah training with the Cantor

Bar/Bat Mitzvah training with a tutor
Rabbinic counseling
Haftorah booklet
Facility use for Saturday morning Kiddush immediately following services
Facility preparation and clean-up

I/We have reviewed the above information and agree to abide by all the above financial
policies and guidelines.
