Email ______________________________ Do you follow us on Facebook? _______ By choosing to consign with Growth Spurt Childrens Consignment Sale and signing this agreement, I agree to the following terms set forth by Growth Spurt:
_____ If I work 2 shifts, I will receive 85% of my sales. If I work 1 shift, I will receive 80% of my sales. If I work NO shifts, I will receive 75% of my sales. _____ A non-refundable registration fee of $10 must be received prior to February 20th, 2013 to confirm my spot in the sale. If registering after this date, I will bring a registration fee of $15 to drop-off. _____ If I am scheduled to work a shift and do not show, $10 will be deducted from my check. _____ Items that do not meet Growth Spurts standards may be pulled from the sales floor at any time during the sale. Any item pulled from the sale will be returned to you at pick-up. _____ Any item tag without a red dot will be sold at 50% off during the half price sale. _____ All items not picked up by 5:00pm on Sun. March 3rd, 2012 will be donated to charity. _____ Items without a tag will be placed in Lost & Found. It is my responsibility to check for and identify any of my items at pick-up. _____ Growth Spurt is NOT responsible for theft, loss, or damage to any of my items. _____ My proceeds check will be mailed to me within 2 weeks after sale. I will provide a self-addressed stamped envelope at drop-off. I will also, include additional postage if requesting that my tags be mailed with my check. If tags are requested and additional postage is not included, $2.00 will be deducted from your check to cover the cost. Do you want your tags mailed to you? ________
