I am an employee of Beta Electric Corporation; That sometime on August 2012, I surrendered to the office of my employer my company-issued ID for purposes of acquiring new company ID for the new project that I will be assigned; That the ID I surrendered was duly received by the staff of my employer; That on the first week of September 2012, I was informed by my office that my ID is no longer to be found; That I have been searching for my ID in the office where of the staff to whom I surrendered my ID but still to no avail; That for all purposes, the aforementioned company ID is considered as lost; That I am attesting the veracity of the foregoing facts for purposes of issuance of another company ID and for whatever purpose this may serve.
