In these cumulative rankings, Interfaith placed 14th out of 19 Health Centers.


Clearly there is room for improvement.

There are a few principles that the management team and Board will be discussing
to guide our efforts to improve quality of care. Those principles include:

• All patients are assigned to a particular provider team.

• Provider teams are accountable for continuously improving the quality of care
and service provided

• Provider teams will be measured on their performance and provided regular


feedback.

• Managers are accountable for improving the performance of our clinic system
as a whole and for creating clinic processes and systems that enhance quality
of care and service.

• Financial incentives for top performing teams
