    Chapter One

    Reye's – The Beginnings

    Between 1951 and 1962 fourteen girls and seven boys between five months and eight and a half years of age were admitted to the Royal Alexander Hospital for Children in Sydney, Australia with a mysterious disease. The first ten were admitted between 1951 and 1959 and all ten died. Of the group of eleven children admitted between 1959 and 1962, 7 died and only 4 survived.

    During those years, Dr. Ralph Douglas Kenneth Reye was the Director of the Institute of Pathology at the Royal Alexander Hospital for Children. Dr. James Baral was Resident Pathologist, and Dr. Graeme Morgan was Chief Resident Medical Officer.

    During Dr. Reye's examination of two of the children admitted to the Royal Alexander Hospital for Children in 1957 who perished, Dr. Reye reported on the autopsy of the second of these that he was seeing a consistent and unusual set of abnormalities. This led to the clinical diagnosis of fatty degeneration of the viscera being made during life in the next patient.

    In the autopsy reports of the first few patients, Douglas Reye noted very diffuse involvement of liver cells with marked fatty degeneration, little evidence of necrosis or inflammation, and an appearance which did not suggest to him an infective process. He noted changes in the kidneys and in the brain and raised the possibility of exposure to some unknown toxin.

    In his report on the autopsy of the fourth patient, a girl aged 6 years and 9 months admitted in 1956, he commented that in each of the affected organs, namely the liver, kidney and cerebrum, damage took the form of a cytoplasmic disorder without any equivalent nuclear disturbance and without any associated vascular or mesoblastic disturbance. He wrote, It seems unlikely that any viable agent, bacterial or viral, would produce a pattern quite like this, and one can only postulate some undiscovered 'toxin' which has caused a disordered metabolism in the cells of the organs concerned.

    In the sixth patient he noted that the myocardium and the pancreas were also affected and at the end of the report on the seventh patient, in November 1957, he wrote: "This is a further example of the visceral and cerebral degenerative disease of unknown etiology
